Fatty liver means extra fat has collected inside your liver cells. The grade on your ultrasound report describes how much fat the radiologist saw. Grade 1 is mild, grade 2 is moderate, grade 3 is severe.
If your report says grade 1 or 2, take a breath. It is extremely common in India, affecting roughly 1 in 3 adults, and at these stages it is usually reversible with steady lifestyle change. It is a warning light, not a verdict.
What is fatty liver?
A healthy liver holds very little fat. When fat crosses about 5 percent of the liver's weight, doctors call it fatty liver. When it happens without heavy drinking, it is called non-alcoholic fatty liver disease, or NAFLD. Alcohol can cause the same picture separately.
The liver quietly runs over 500 jobs, from filtering blood to storing energy. Extra fat makes every one of those jobs harder.
What do the fatty liver grades mean?
| Grade | Fat level | What it usually means | What to do |
|---|---|---|---|
| Grade 1 | Mild fat build-up | Earliest stage, liver works normally, usually no symptoms | Fix diet and activity now, fully reversible for most |
| Grade 2 | Moderate fat build-up | The process is advancing, liver tests may rise | Act seriously, doctor review plus firm lifestyle change |
| Grade 3 | Severe fat build-up | High load, risk of inflammation and scarring | Needs proper medical management |
Is grade 1 fatty liver dangerous?
Not immediately. Grade 1 fatty liver does not mean liver damage, and most people feel nothing at all. The danger is only in ignoring it. Over the years, fat can trigger inflammation, called NASH, and inflammation can slowly turn into scarring, called fibrosis. Scarring is much harder to undo.
So treat grade 1 as your cheapest exit. Caught here, the liver can return fully to normal, often within months.
Is grade 2 fatty liver dangerous?
Grade 2 deserves respect. The fat load is higher, liver enzymes like SGPT often start rising, and the risk of inflammation grows. It is still reversible for many people, but half-hearted effort will not do it. This is the stage to involve your doctor, fix the routine properly, and retest in 3 to 6 months.
Fatty liver symptoms: why most people feel nothing
Fatty liver is mostly silent. When signs appear, they are vague:
- Tiredness that does not match your day
- Heaviness or dull ache under the right ribs
- Mild bloating after meals
- Slowly rising SGPT and SGOT in blood reports
SGPT is a liver enzyme, a protein that leaks into blood when liver cells are stressed. Most labs keep the upper limit around 35 to 45 U/L. A mildly high SGPT with a fatty liver ultrasound is a very common combination. Most cases are picked up exactly this way, on a routine health check.
What causes fatty liver?
- Belly weight and overall weight gain
- Insulin resistance, where cells respond poorly to insulin and the liver stores the excess as fat
- Too much sugar, maida and sugary drinks
- Sitting most of the day
- Regular alcohol
- Diabetes, high triglycerides and thyroid imbalance
Because insulin resistance drives it, fatty liver and rising blood sugar usually arrive together. Check where you stand with our blood sugar levels chart by age. Thin people can get fatty liver too, especially with belly fat and a sweet tooth.
How to reduce fatty liver: the five steps that actually work
- Lose 5 to 10 percent of body weight. This is the single best-proven step. At 80 kg, that means 4 to 8 kg, and liver fat drops sharply.
- Cut sugar and sugary drinks to zero. The liver converts excess sugar, especially from drinks and sweets, straight into liver fat.
- Move 150 minutes a week. Brisk walking works. Add two strength sessions, because muscle burns sugar before the liver has to store it.
- Stop or strictly limit alcohol. A fatty liver has no spare capacity for it.
- Sleep 7 to 8 hours. Poor sleep worsens insulin resistance and appetite.
Follow this consistently and grade 1 often improves within 3 to 6 months. Retest with an ultrasound and liver profile to see the proof. Unsweetened coffee, one to two cups a day, is also linked with better liver health in research.
A simple fatty liver diet, in one line
Home-cooked plates built on vegetables, dals, whole grains and lean protein, with fried food, sweets and packaged snacks kept for rare occasions. Nothing exotic. Boring consistency is what shrinks liver fat.

Frequently asked questions
Can grade 1 fatty liver disappear completely?
Yes. With 5 to 10 percent weight loss and a clean routine, many people see a normal ultrasound within 3 to 6 months.
How long does it take to reverse grade 2 fatty liver?
Usually 6 to 12 months of consistent effort, sometimes longer. Track progress with repeat ultrasounds and liver function tests under your doctor.
Is fatty liver the same as liver damage?
No. Grades 1 and 2 mean fat storage, not damage. Damage begins only if long-standing fat causes inflammation and scarring.
Which tests confirm fatty liver?
An abdominal ultrasound spots and grades it. A liver function test checks enzymes like SGPT and SGOT. Doctors sometimes add a FibroScan to measure stiffness.
Does fatty liver cause pain?
Usually not. Some people feel heaviness under the right ribs. Sharp or severe pain points to something else and needs a doctor.
